Reactivation Application for CRN-BC™
(Emeritus returning to Clinical Research Profession)

Application Type:

It is the responsibility of the individual CRN to notify the Clinical Research Nurse Certification Council (CRNCC) of changes to their name and/or address. CRNCC is not responsible for notices that fail to reach the Clinical Research Nurse (CRN).

I attest to having a current unencumbered RN license or country equivalent

Application to reactivate CRN-BC™: I attest to having an active CRN-BC™ Emeritus designation for less than three (3) years, at the time of this request, and wish to reactivate my CRN-BC™.  (The date of Emeritus designation is located on the certificate).

I attest to having completed fifteen (15) contact hours of continuing education (CE) for each year of Emeritus designation preceding this request for reactivation; with at least 10 of those 15 required hours (per year) earned in clinical research.

 I attest that I am familiar with the following:  

  • The CRN-BC™ Emeritus credential requirements for CRN-BC™ certification reinstatement.

  • After three (3) years of Emeritus status, CRNs desirous of holding an active CRN-BC™ credential must reapply by fulfilling the initial certification qualifications and following the initial portfolio submission process described in the CRN Certification by Portfolio Candidate Handbook.

My submission affirms that all information submitted for the purpose of reinstatement consideration is accurate and complete. It is my understanding that the finding of this application for reinstatement is based solely on the materials I submit and will be denied for any of the following reasons:

 

Falsification of application; Misrepresentation; Failure to meet criteria for contact hours/points per policy; Failure to provide requested information/documentation requested during audit; Failure to meet experiential requirement; Lack of current unencumbered RN license; Failure to submit appropriate fees; Failure to apply by deadline.
